1. About L'Art de Vie

L'Art de Vie ("L'Art de Vie," "we," "us," or "our") provides personalized functional medicine, wellness, and health optimization services through telehealth under the direction of Kelly Clark, PA-C.

Our services include no-lab and lab-based consultations — Weight & Metabolic Health, Pain, Recovery & Resilience, Energy & Vitality, Women's Hormone Health, and Healthy Aging & Longevity — as well as coordination of laboratory panels, personalized wellness protocols, health coaching, ongoing monitoring, and, where clinically appropriate, prescriptions such as GLP-1 therapy or peptide therapy.

Our website provides information about our programs and allows eligible individuals to request consultations and related services.

3. Eligibility

Cancellations and No-Shows

Cancellation, rescheduling, and no-show requirements will be disclosed during booking or in your appointment confirmation. Appointments cancelled without sufficient notice or missed without notification may result in a cancellation or no-show fee.

Appointment Preparation

You may be required to complete intake forms, provide identification, disclose your current location, submit medical records, or complete laboratory testing before your consultation. Late arrivals may result in a shortened appointment, cancellation, or rescheduling.

5. Telehealth Services

L'Art de Vie services are provided through video calls, telephone calls, electronic forms, secure messaging, or other telehealth technologies. By participating in telehealth services, you understand that:

• Telehealth may have limitations compared with an in-person examination.
• Technical issues may interrupt or delay a consultation.
• A provider may require additional testing or an in-person evaluation.
• You are responsible for using a private location and secure internet connection.
• You must not record consultations without prior permission.

A separate telehealth consent form may be required before services are provided.

6. Payments and Fees

Payment may be required before a consultation, laboratory service, or other service is provided. By providing payment information, you authorize L'Art de Vie and its payment processors to charge the applicable fees.

Prices may vary depending on your selected program, provider recommendations, laboratory requirements, medications, and other services. Prices may change for future services without prior notice. You are responsible for any fees charged by your bank or payment processor. Refunds, where available, will be governed by the refund or cancellation policy disclosed at the time of purchase.

7. Insurance

Unless expressly stated otherwise, L'Art de Vie services may not be covered by health insurance, Medicare, or Medicaid. You are responsible for confirming whether your insurance provider offers reimbursement. L'Art de Vie does not guarantee insurance coverage or reimbursement, and you remain responsible for all applicable fees.

8. Health Information and Informed Consent

Before receiving services, you may be required to complete a health and medical-history questionnaire, disclose current medications and conditions, provide laboratory results, review and sign informed-consent documents, and confirm your identity and physical location. You represent that all information you provide is truthful, current, and complete. Incomplete or inaccurate information may affect the safety and effectiveness of your care.

9. Laboratory Testing

L'Art de Vie may coordinate laboratory testing through independent laboratories and licensed service providers. You are responsible for following all preparation, collection, storage, and shipping instructions. Testing or processing may be delayed due to insufficient samples, collection errors, shipping delays, laboratory issues, or circumstances outside our control. Laboratory results should be reviewed with a qualified healthcare provider and not used for self-diagnosis.

10. Prescriptions and Clinical Decisions

All clinical decisions are made by licensed healthcare providers using their independent professional judgment. Completing a consultation does not guarantee that you will receive a prescription, medication, laboratory order, or particular treatment. A provider may determine that a requested service is not clinically appropriate, that additional testing is required, that an in-person evaluation is necessary, or that you should seek care from another provider. Any prescription must be used only as directed by the prescribing provider and dispensing pharmacy.

11. Compounded and Off-Label Medications

Certain medications recommended through L'Art de Vie — including some GLP-1 therapies or peptide therapies — may be compounded by licensed pharmacies or prescribed for an off-label use when a licensed provider determines that doing so is clinically appropriate. Compounded medications are not reviewed or approved by the FDA in the same manner as commercially manufactured, FDA-approved medications. The prescribing provider or pharmacy will provide information regarding benefits, risks, alternatives, dosage, and proper use.

When a medication is prescribed through L'Art de Vie, it is dispensed by a pharmacy licensed and based in the United States. Specific pharmacy information, including the dispensing pharmacy's name and contact details, is provided to you at the time your prescription is processed.

12. No Guarantees of Results

Individual results vary based on many factors, including age, medical history, genetics, lifestyle, nutrition, sleep, health conditions, and adherence to provider recommendations. L'Art de Vie does not guarantee any specific health, wellness, weight, longevity, laboratory, or treatment outcome.

13. Third-Party Providers

L'Art de Vie may coordinate services with independent laboratories, pharmacies, technology providers, scheduling platforms, payment processors, or other third parties. These providers may have their own terms, fees, processing times, and policies. L'Art de Vie is not responsible for the actions, delays, errors, or services of independent third-party providers where those matters are outside our reasonable control.

14. Intellectual Property
